Summary:
Responsibility for the management and support of an ACO team and project staff. Provides leadership for the development and administration of an evidence-based population health plan, including undertaking local needs analysis, program and service delivery. Accountable for measured success in the improved health of assigned various patient populations (Medicare, Medicaid, commercial, and uninsured) as evidenced by patient-activated self-care. Accountable for positive outcomes measures, e.g. ACO measures.

Education:
Essential:
* Bachelor's degree in related field.

Experience:
* 10 years of related experience in care coordination, integrated network, and accountable care organization
* Staff management experience.
