Summary:
Provides specialized clinical activities relating to designated clinical programs in order to ensure optimal patient outcomes. Performs various types of medication utilization activities within a defined area, including but not limited to anticoagulation, pharmacokinetics, dose adjustments, order verification, and IV to PO-conversions. Participates in committees and represents the department of pharmacy throughout the hospital or ambulatory settings within the area of focus.
This position is focused on the pediatric hematology/oncology service line, covering both the inpatient acute care unit and outpatient infusion center. Along with rounding and maintenance of Children’s Oncology Group (COG) roadmaps and protocols, the ideal candidate will actively support the system’s cost avoidance initiatives as well as investigational drug dispensing. Responsible for maintenance of Epic Beacon treatment and therapy plans in close liaison with the pharmacy informatics team. The successful applicant will also contribute to the pharmacy department’s clinical activities, including but not limited to, precepting, presenting CE’s, representing committees.
