Summary:

Supports the Director of Care Coordination in the monitoring and process of organizing patient's care across multiple healthcare providers to ensure that the patient's needs and healthcare preferences are met. Tracks all quality measures. Ensures that staff is providing the highest quality services in the most efficient, cost-effective manner. Implements process improvement and clinical integration protocols. Responsible for overseeing and managing all aspects of HEDIS population health, quality indicators management, develop and execute quality plans, policies and procedures to ensured adherence to quality standards and payors quality meetings. Monitor and improve quality outcomes. Analyze quality data identify areas for improvement corrective actions. Collaborate with other departments to ensure quality is integrated into ambulatory division practice.
