Summary:
Provides specialized clinical activities relating to designated clinical programs in order to ensure optimal patient outcomes. Performs various types of medication utilization activities within a defined area, including but not limited to anticoagulation, pharmacokinetics, dose adjustments, order verification, and IV to PO-conversions. Participates in committees and represents the department of pharmacy throughout the hospital or ambulatory settings within the area of focus.

This position will be focused on medication safety practices and initiatives. The ideal candidate will have substantial knowledge of hospital pharmacy and safety practices. Critical thinking and problem solving skills will ensure a successful tenure.

Minimum of 2 years Pharmacist experience to be eligible for 20K bonus.
