Summary:
Manages the ongoing, effective operation of the functions for case management and social services. These functions include resource/utilization management and discharge planning and incorporates linkages with the community and the various levels of care provided by the North Broward Hospital District.

MUST BE A REGISTERED NURSE.

Education:
* Bachelor's degree required. Master's preferred.
* Graduate from an accredited nursing school (AD in Nursing, RN Diploma, BSN or MSN).

Experience:
* Five years of management/clinical experience with at least three (3) years relevant case management experience.
* Knowledge of clinical data interface, InterQual, Case Management documentation, secure faxing, Avoidable Day tracking.

Credentials:
* Licensed as a Registered Nurse in the State of Florida or holds a multi-state nursing license; or able to obtain license prior to start is required.
* Certified Case Manager preferred.
