FULL-TIME – MONDAY – FRIDAY – DAYS – WEEKEND REQUIREMENTS: AS NEEDED


Job Qualifications

• Bachelor’s degree in Social Work or related field

• Two years of related experience required

• American Heart Association (AHA) BLS Provider card

Job Summary

Assists families in the coordination of appointments with physicians, as well as another necessary referrals to specialists and/or medical tests. Arranges for developmental and nutritional assessments and addresses any psychosocial needs, helping families navigate systems for financial and social assistance. Ensures that families participate in the treatment, planning and continuous care of their children by establishing an initial Family Support Plan (FSP) which is reviewed and updated every six months, or as often as needed for more medically complex children.
