Provides discharge planning services to patients at risk. Conducts biopsychosocial interviews, arranges, coordinates, and advocates for services for patients for the purpose of resolving difficult situations through the maximum use of community resources.

Case management experience highly desired 

Position is In Person – No Remote Work.

Education:
* Bachelor of Social Work

Experience:
* One Year

*Level II Background Screening required
