“Broward Health is Broward County’s largest healthcare services provider and is one of the nation’s top public health systems. We are seeking a qualified professional to join our Ambulatory Pharmacy Division.

Role Description

This is a full-time on-site role located in Fort Lauderdale, FL, for a Clinical Pharmacy Coordinator within the Ambulatory Division- Broward HealthPoint. The Clinical Pharmacy Coordinator plays a crucial role in both direct patient care and administrative leadership, with working primarily in direct patient care (70-80%), with additional administrative responsibilities (20-30%). The ideal candidate will provide expert medication management and lead key pharmacy initiatives to optimize patient outcomes.

Key Responsibilities

Conducts comprehensive medication reviews and assessments, collaborates with healthcare teams to develop individualized treatment plans, manages the Anticoagulation Monitoring Service, and educates patients and caregivers on medication regimens, side effects, and adherence strategies. In addition to patient care, the coordinator oversees the clinical work of pharmacists and specialists within the Ambulatory Pharmacy Division, leading service line development, medication selection, and policy creation to enhance pharmacy practice. They ensure safe, cost-effective drug therapy and optimal patient outcomes while also chairing assigned committees and coordinating drug program initiatives. Furthermore, the role involves supporting and implementing Joint Commission and regulatory initiatives, as well as upholding medication management standards.

Qualifications

  • State licensure as a Pharmacist in Florida
  • Doctor of Pharmacy (Pharm.D.)
  • Board certification and/ or completion of a residency program preferred
  • Experience in a clinical pharmacy setting is preferred
  • Excellent organizational and communication skills
  • Ability to work collaboratively within a healthcare team
