Summary:
Provides day-to-day business support and defines the scope of project objectives. Conducts or participates in projects and tactical meetings and handles issue tracking and analysis. Ensures adherence to quality standards and reviews deliverables with customers.

The Epic Healthy Planet Analyst is responsible for the analysis, design, implementation, support, and optimization of the Epic Healthy Planet module, which supports population health management, care coordination, value-based care, and risk stratification initiatives. This role partners clinical, operational, and IT stakeholders to drive improvements in health outcomes and care delivery through data-driven insights and workflows.

This is an onsite position in Fort Lauderdale, FL.


Key Responsibilities:

  • Lead the configuration, build, test, and maintenance of Epic Healthy Planet applications, including care gaps, registries, dashboards, and worklists.
  • Collaborate with clinical and population health teams to define and design workflows that support care management, social determinants of health (SDOH), and quality improvement programs.
  • Drive the design and optimization of workflows while developing and maintaining robust registries for patient cohorts, including risk-based stratification and attribution logic.
  • Serve as a primary liaison between clinical users and technical teams, effectively gathering requirements, translating complex needs into technical specifications, and delivering user-centric solutions.
  • Develop analytics and reporting tools using Slicer Dicer, Reporting Workbench, and Radar Dashboards to support care teams and leadership.
  • Support and optimize workflows for chronic disease management, preventive care, annual wellness visits, and other population health initiatives.
  • Monitor application performance and assist in issue resolution and root cause analysis.
  • Contribute significantly to upgrading testing, validation, and go-lives.
  • Ensure system compliance with regulatory and accreditation standards (e.g., HEDIS, CMS, NCQA).
  • Troubleshoot complex issues related to Epic Healthy Planet, providing timely resolution and root cause analysis to ensure system stability and performance.
  • Provide user training and develop comprehensive documentation as needed to support end-users.

Qualifications:

Required:

  • Bachelor’s degree in Information Systems, Computer Science
  • Epic Healthy Planet certification (or ability to obtain within 3 months).
  • Minimum of 3+ years of progressive experience in Epic clinical or population health applications, specifically with a focus on Healthy Planet module implementation and support.
  • Strong understanding of population health principles, risk stratification, and value-based care models.
  • Experience in clinical workflows, care management, or quality improvement initiatives.

  • Healthy Planet and Compass Rose Clinical Build
  • Healthy Planet Value-Based Analytics Build
  • Demonstrated proficiency with Epic reporting tools, including Slicer Dicer, Reporting Workbench, Radar, and/or Clarity/Caboodle.
  • Exceptional analytical and problem-solving skills, with the ability to interpret complex data and translate it into actionable solutions.
  • In-depth understanding of population health principles, risk stratification methodologies, and value-based care models (e.g., ACO, CIN, PCMH)
  • Proven experience in optimizing clinical workflows, care management programs, and quality improvement initiatives within a healthcare setting.
  • Exceptional analytical and problem-solving skills, with the ability to interpret complex data and translate it into actionable solutions.
  • Strong communication and interpersonal skills, capable of building effective relationships with diverse stakeholders (clinical, technical, operational).
